Aggressive. As a bonus action, the scabbie can move up to its speed towards a hostile target it can see.
Blessing of Yurtrus. Any spell the scabbie casts that deals radiant damage deals necrotic damage instead.
Spellcasting. The scabbie is a 5th-level spellcaster. Its spellcasting ability is Wisdom (spell save DC 13, +5 to hit with spell attacks). The scabbie has the following cleric spells prepared:
Cantrips (at will): resistance, sacred flame, toll the
dead, word of radiance
1st level (4 slots): bane, guiding bolt, inflict
wounds, protection from evil and good
2nd level (3 slots): blindness/deafness, spiritual
weapon
3rd level (2 slots): bestow curse, dispel magicSubdermal Parasites. A creature that touches the scabbie or hits it with a melee attack while within 5 feet of it must make a DC 13 Constitution saving throw.
On a failed save, the target is infected by subdermal parasites and must succeed on a DC 13 Wisdom saving throw at the start of each of its turns or spend its action that turn scratching the relentless itching of the parasites. A creature that fails its saving throw by 5 or more also takes 5 (2d4) slashing damage from their own scratching as they gouge out chunks of their own flesh. The parasites are destroyed if the target receives magical healing.
Additionally, any creature can take an action to treat the parasites with a successful DC 13 Wisdom (Medicine) check. On a failure, the creature attempting to treat the parasites is also infected.Actions
Tentacles. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 6 (1d8 + 2) bludgeoning damage plus 4 (1d8) necrotic damage. If the target is a creature, it is affected by the orc's subdermal parasites.

Initiates in the cult of Yurtrus are not immediately entrusted with the spreading of its signature diseases. There is an induction process that involves working your way up to being a harbinger of death.
Infestation. The first of these steps is to test the resolve of new members. This involves less infection and more infestation, as the cult introduces parasites into the initiates body. These parasites burrow into the flesh and are rapidly spread by physical contact. Yurtrus influence then spreads throughout the body of the host, as well as other physical changes induced by the hosts own obsessive scratching and gouging, to alleviate the itching symptoms of the parasites.
The scabbie is then sent out from the cult to a nearby town or city, to spread their infestation as far as possible. The cult monitor their progress in secret and will advance their cult status if they are happy with the results.
