Codex of the Infinite Planes

The Codex is a massive tome, far larger than any book has a right to be. It is said that two strong men can barely lift the volume. The covers are made of flawless black obsidian and the pages therein are sheets of lead hammered so thin that they flex like paper. These are illuminated with strange writings in languages unknown and illustrations both beautiful and horrific. No matter how many pages are turned, there is always one more.

While the Codex is massively heavy to anyone not attuned to it, once you become attuned you can carry it around and weighs no more than a heavy tome.

Random Properties. The Codex of the Infinite Planes has the following random properties:


  • • 3 minor beneficial properties
  • • 1 major beneficial property
  • • 3 minor detrimental properties
  • • 2 major detrimental properties

Ability Score Improvement. While attuned, you can increase one ability score by 2 or increase two ability scores by l each. The ability score can now be increased above 20, up to a maximum of 24.

Invoked. All powers of the Codex are triggered by reading, if one knows where to look. The Codex can open a portal to any plane, demiplane, or prime material world at any location. The book, however, has no index or table of contents. To open a portal, you must succeed on a DC 15 Intelligence check. On a successful check, you cast the gate spell to your desired location. On a failure, you and each creature and object within 15 feet of you travel to a random plane of existence as if under the effects of a plane shift spell, though no saving throw can negate. You can use this effect once per day.

Summon Greater Demon. You cast the Summon Greater Demon spell as if it were cast as a 9th level spell through the book, this does not expend a spell slot and the Demon is under your control for 1 hour. This does not require concentration.

Ill-Fated. Every time it's powers are used, the reader has a 1% cumulative chance of triggering an awful fate (by accidentally opening the wrong portal)—this could be indefinite madness, the arrival of a tarrasque, 10-mile radius clouds of deadly poison (no saving throw), or worse. No character can read more than 99 pages before doom befalls them.

Destroying the Codex. Some say the only way to destroy the codex is to read all of its infinite pages or to destroy all of the planes.

The Moaning Diamond

Said to have been ripped from the ground in a ritual that tortured the earth itself, the Moaning Diamond appears to be an uncut diamond the size of a human fist. At all times, it gives forth a baleful moaning sound, as if in pain. Despite the noise, the Moaning Diamond is not evil (although it was birthed in torture).

Random Properties. The Moaning Dimaond has the following random properties:


  • • 2 minor beneficial properties
  • • 1 minor detrimental property
  • • 1 major detrimental property

Spells. The diamond has 7 charges and regains 1d4 + 3 expended charges daily at dawn. If you control the diamond, you can use an action and expend 1 or more charges to cast one of the following spells (save DC 18) from it: stone shape (2 charges, affects up to 500 cubic feet of stone), Wall of Stone (3 charges), stoneskin (2 charges), or investiture of stone (4 charges).

Elemental Command. As an action, the Moaning Diamond can summon an earth elemental myrmidon with maximum hit points that serves the caster until it is slain or the caster is no longer attuned to the Moaning Diamond, whichever happens first. Only one such elemental can be summoned at a time; if it is slain, a new creature cannot be summoned for 24 hours.

Protection from Elementals. While attuned to this diamond, all elementals have disadvantage on attack rolls against you as if a protection from evil and good spell was cast on you.

Destroying the Diamond. To destroy the diamond, you must throw it into the heart of a volcano and it must stay there for 100 years before it gives way to the intense heat and flames, destroying it forever.

Amulet of Passage

This fine silver chain is unassuming, bearing a single arrowhead shaped jewel. While you wear this amulet, it stirs thoughts of legendary battles and heroic deeds.

Random Properties. The Moaning Dimaond has the following random properties:


  • • 2 minor beneficial properties
  • • 1 minor detrimental property
  • • 1 major detrimental property

Indombitable. While attuned to this amulet, you gain a +2 bonus to all saving throws.

Unstoppable. While you wear this amulet, difficult terrain doesn't cost you extra Movement. In addition, magic can neither reduce your speed nor cause you to be Paralyzed or Restrained.

Furthermore, you have advantage on all Acrobatics, Athletics and Thieves' Tools checks.

Increased Movement. Your walking speed increases by 30 feet.

Teleporation. While attuned to the amulet, you can select any number of creatures within 30 feet of you to gain the ability to cast the misty step spell once per long rest. To do so, the creature must begin within 30 feet of you, and teleport to a location within 30 feet of you.

Destroying the Amulet. To destroy the amulet, you must set it between the titanic gears of Mechanus, this will turn it into a find powder. If any fragments escape turning into a powder, it will grow back to its full size and power in 1d100 years.
