Mutated Form (Mythic Trait; Recharges after a Long Rest). If Lady D. is reduced to 0 hit points, she doesn't die or fall unconscious. Instead, she regains 300 hit points and maximum hit points, as well as gaining movement abilities and more attacks.

Regeneration. Lady D. regains 25 HP at the start of its turn if it has at least 1 HP.

Aura of bats (mutated form only). Every creature that starts its turn within 15 ft of Lady D. must make a DC 16 dexterity saving throw. A creature takes 10 (4d4) piercing damage on a failed save or half as much on a successful one.

Legendary resistance (2/day). If lady D. fails a saving throw, she can choose to succeed instead.

Exposed humanoid form (mutated form only). If a creature hits Lady D. with an attack roll that exceeds 24 with bonuses added, her human form is hit, causing the attack to deal double damage.

Actions

Claw attack. Melee weapon attack: +10 to hit, reach 10 ft, up to two creatures within range, Hit: 23 (5d6+6) piercing damage.

Tail attack (mutated form only). Melee weapon attack: +10 to hit, reach: 15 ft, up to 3 targets within range. Hit: 15 (2d8+6) bludgeoning damage.

Reap (mutated form only). Lady D. attacks with it's mutated mouth devouring 1 medium or smaller creature. One creature within 5 ft of Lady D must make a DC 16 dexterity saving throw or take 25 (7d6) piercing damage and become grappled (Escape DC 16) and restrained until the grapple ends. While grappling a creature, Lady D. cannot attack with Reap.

Deadly Leap (recharge 5-6) (mutated form only). If Lady D. covers at least 30 feet of horizontal distance with her jump, she may land on a tile that 1 or more smaller creatures occupy. All creatures under Lady D. must make a DC 16 dexterity saving throw, on a failure they takes 18 (4d8) bludgeoning damage and are knocked prone and remain prone until Lady D. moves away so she isn't on top of them. On a success a creature moves to a tile adjacent to Lady D.
