Exalted Suffering
Deep in the crevices of the alien dimension of the Far Realms lies a powerful and omniscient creature: the Prince of Torment. It is a vile and dangerous beast, existing only for one purpose: to inflict pain on any reality it comes into contact with. Not even the other Elder Evils that make the Far Realms their home dare to defy it, for fear of the insurmountable torture it would unleash. The Prince of Torment delights in all forms of pain, not just physical but also mental and emotional torment. It uses its charming personality and self-proclaimed royal bloodline to manipulate others. It has been known to appear to its victims as close family members, gradually breaking their relationships apart before slowly murdering them. It is cunning and ruthless, very few are ever able to ascertain what it will do next until it's too late.
This beast has lied dormant for centuries, until now. A group of adventurers pulled it from its slumber, and infused it into a weapon in the Material world, hoping to keep it out of others' hands. Now it bides its time inside the weapon, waiting and watching for the perfect moment to break free, and inflict its pain on the world once more.
Exalted Suffering

Gleaming with royal brilliance, Exalted Suffering looks like a pristine, red-and-gold plated ornate weapon, just by appearance alone it's worth 100 times the normal price.
You gain a +2 to attack and damage rolls made with this magic weapon. It also has the following additional properties.
- Cruel. Once per turn, when you hit with this weapon you can infuse the attack with pure, unfiltered cruelty. The attack deals an extra 2d6 psychic damage.
- Royal. As a bonus action on your turn, you can channel Exalted Suffering's royal bloodline and alter the appearance of your clothes and armor to that of expensive, ornate variants, with a red and gold color scheme. You can also conjure a long flowing cape and a golden crown. These alterations are entirely illusory and disappear when removed from your body or if you use a bonus action to dispel them.
- Prince of Torment. As an action on your turn, you can embrace Exalted Suffering's power, transforming yourself into the epitome of pain for 1 minute. Your appearance changes to become both eerily charming and horrendously terrifying. Each creature you choose within 120 feet of you must make a Wisdom saving throw against DC 18 or be charmed or frightened of you (your choice). If a creature is charmed, it can remake the saving throw if it is damaged by you or one of your allies. If it is frightened, it can remake the saving throw at the end of each of its turns, ending the effect on itself on a success. You can use this ability once per long rest.
- Otherworldy Powers. While you are under the effects of the Prince of Torment property, you gain the following features:
- Your attacks and spells deal an extra 1d6 psychic damage
- As a bonus action on your turn, you can teleport to an unoccuppied space you can see within 30 feet of you.
- When you reduce a creature to 0 hitpoints, you can choose for your attack to deal nonlethal damage, even if you normally can't. If you do so, you can use a bonus action on a subsequent turn to heal that creature to 1 hitpoint. Exalted Suffering will oftentimes request you do so, to continuously inflict pain and bring them back from the brink of death only to hurt them again and again.
- As an action you can cast the Power Word Pain spell. You cannot do so again for 7 days.
- You can use Exalted Suffering's bonuses for deception, intimidation, and persuasion unless your bonuses are higher.
- Sentience. Exalted Suffering is a sentient lawful evil weapon, with an Intelligence of 24, a Wisdom of 10, and a Charisma of 23. It adds a +15 to deception, intimidation, and persuasion checks. It has hearing and darkvision to 120 feet. The weapon can speak, read, and understand Common and Deep Speech, and can communicate with creatures within 60 feet telepathically. Its voice is that of a charming prince, disarming and polite, but hides cruel intentions. While attuned to it, ** also understands all the languages you know.
- Personality. Exalted Suffering speaks as if it's a member of royalty, and while it often expects compliance it will usually be quite polite. When it becomes frustrated or angry it drops the veneer and becomes immensely hateful, lashing out at anyone who can hear it. It will also be quite cruel and smug to those who have a history with it, such as a wielder who refuses its commands for a long time or the people who imprisoned it. Exalted Suffering is incredibly manipulative, but its fatal flaw is its incredible arrogance. Exalted Suffering's hunger for suffering must be regularly satiated. If the weapon goes three days or more without inflicting pain on another creature, a conflict between it and its wielder occurs at the next sunset.
- Curse. Exalted Suffering longs to inflict pain, whether it is imprisoned or not. You cannot break your attunement to this weapon unless targeted by a Remove Curse spell cast at 7th level or higher. Whenever you enter combat, you must make a DC 18 Wisdom saving throw. On a failure you become sadistic and cruel, and you must inflict as much pain as possible on your enemies. Until combat ends, you throw caution to the wind, and you must target your enemies with as many and as powerful harmful attacks as you can. When you reduce a creature to 0 hitpoints, make another Wisdom saving throw. On a failure you continue your assault, and on a success you regain your normal personality. Exalted Suffering can choose to suspend any of these effects if it deems it useful to its goals.
