Destrachan

This creature appears like a blind tyrannosaurus rex with the circular mouth of a lamprey eel. It stalks through dark tunnels, hunting down prey using its acute senses where it then bellows and screeches, causing eardrums to burst and minds to be torn asunder.

Appeared originally in Dungeons & Dragons 3e & 4e. More information, Forgotten Realms Wiki.

Actions

Multiattack. The destrachan makes either three attacks: one with its bite and two with its claws.

Bite. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 16 (2d10 + 5) piercing damage plus 9 (2d8) thunder damage.

Claws. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 12 (2d6 + 5) slashing damage.

Bellowing Burst (Recharge 5-6). The destrachan bellows with thunderous noise in a 30-foot cone. Each creature in that area must make a DC 14 Constitution saving throw, taking 21 (6d6) thunder damage on a failed save, or half as much damage on a successful one.

Reactions

Reverberate. If the destrachan must make a saving throw against thunder damage, it can reverberate in an opposite frequency. It gains advantage on the saving throw and all creatures within 10 feet of it have disadvantage on the save.
