Bel'Veth, the Empress Void
Anightmarish empress created from the raw material of an entire devoured city, Bel’Veth is the the beginning of a monstrous reality of her own design.
Driven by epochs of repurposed history, knowledge, and memories from the world above, she voraciously feeds an ever-expanding need for new experiences and emotions, consuming all that crosses her path.
Transforming Bel'Veth
Bel'Veth has two forms, the fake (start) and the true (legendary), each one have different rules. The fake is a simply monster of the Manual, nothing complex, but the legendary have homebrew rules.
Epic Transformation. Try to make the transition epic and, probably, mortal (haha). Fake-Bel'Veth is a less difficult of a normal Challenge 11, but if she take the Coral, the battle will turn and be hard.
True Form Hit Points. When transformed, his Hit Point sapare to General Hit Points and Individual Limbs Hit Points, that can dissable some abilities of Bel'Veth when they zero. All damage deal to Fake Form, comes to General Hit Points.

Death in Lavander. When a creature dies 30 ft. near Bel'Veth, it drops a Coral. If Bel'Veth take it, she transforms into Bel'Veth, True Form (next page) until the end of combat.

Slap. Melee Natural Weapon Attack: +6 (STR + Proficiency) to hit, 5 ft., one target. Hit 2d6+2 (4) piercing damage
Royal Maelstrom. Bel'Veth does six Slap in closest creature of she. If it dies, she take the Coral instantly. (Recharge 6)

Abilities
Especial Hit Points. When a player say clearly he will attack a especial part of Bel'Veth, the damage deals on General and Limps. If Wings zero, Bel'Veth lose half of her fly speed.
Actions
Multiattack. Bel'Veth can make four Slap or two Slap and a Mouth each turn. If Wings Hit Points come to zero, she loses one Slap in both chooses.
Slap. Melee Natural Weapon Attack: +9 (STR + Proficiency) to hit, 5 ft., one target. Hit 2d6+4 (10) piercing damage
Mouth. Melee Natural Weapon Attack: +9 (STR + Proficiency) to hit, 5 ft., one target. Hit 3d8+4 (16) piercing damage. Heals Mouth Hit Points equal the damage deals. If Mouth zero, Bel'Veth lose this action.

Why not legendary actions?
I do this thinking to be a 12th-level four character encountrer. The Fake Form is easier to fight, but if she take-off a character and take the Coral, the players feelings will be "Ow fuc....", but she still be a winnable fight.
More, I love DnD 5e but don't play so long, so I have less experience that I fancy.
