Sanguinare Vampiris

If you want vampirism to run more rampant within your games, then consider using the Sanguinare Vampiris optional rule, which can potentially turn players into vampires! Although true vampires create other true vampires when their own blood is drunk by a Vampire Spawn, the curse of vampirism still affects those with weaker constitutions.

If a creature with blood is bitten by a creature with ‘Vampire’ in its name, they must succeed on DC 14 Constitution saving throw or contract Stage 1 Sanguinare Vampiris. Once a creature has Sanguinare Vampiris, their condition can progress every time they take a long rest.


Stage 1 Sanguinare Vampiris

  The creature feels sick, sweats excessively, and is frequently dehydrated. During this stage, the creature is considered Poisoned.

This stage of Sanguinare Vampiris can be cured by removing the disease or curse via the Lesser Restoration, Protection from Good and Evil, Remove Curse, Greater Restoration, Dispel Evil and Good, or by a similar spell or magical effect. If a creature wishes to resist progressing to Stage 2, at the end of their long rest they can attempt a DC14 Constitution saving throw. On a success, they are not cured of Sanguinare Vampiris, but it does not progress.

Stage 2 Sanguinare Vampiris

  After completing a long rest, the creature’s hit point maximum is reduced by 3d10. If this reduces a creature to zero hit points, they permanently become a Vampire Spawn. This effect is only reversible through a Wish spell or through a special 8 hour Ritual that involves 2,000gp worth of powdered silver and holy water alongside the casting of Dispel Evil and Good.

If the creature is not reduced to zero hit points, they are no longer poisoned, and instead have noticeably paler skin, their muscle mass begins to fade, their eyes begin to lose their original color, and they are more sensitive to light. The creature gains the Sunlight Sensitivity trait and can see in dim light and darkness as if it were bright light out to 60 feet.

This stage of Sanguinare Vampiris can be cured by the spell Remove Curse, Greater Restoration, or Dispel Evil and Good. If a creature wishes to resist progressing to Stage 3, at the end of their long rest they can attempt a DC15 Constitution saving throw. On a success, they are not cured of Sanguinare Vampiris, but it does not progress.






















Stage 3 Sanguinare Vampiris

  The creature eyes’ gain a red tint, they no longer feel the need to sleep or rest, and they lose their sense of taste. The creature gains the benefits of a long rest by spending 6 hours of doing light activity, the creature can no longer sleep, and the creature can no longer taste and gains no nutrition from food or drink, but cannot starve or die from dehydration. Additionally, they can now see in dim light and darkness as if it were bright light out to 120 feet.

This stage of Sanguinare Vampiris can be cured by the spell Greater Restoration or Dispel Evil and Good. If a creature wishes to resist progressing to Stage 4, at the end of their long rest, they can attempt a DC18 Constitution saving throw. On a success, they are not cured of Sanguinare Vampiris, but it does not progress.

Stage 4 Sanguinare Vampiris

  The creature’s hit point maximum is no longer reduced, their nails and teeth begin to sharpen, their skin grows cold, and they feel a thirst for blood. The creature’s creature type changes to Undead, they can now starve or die from dehydration, but this can only be ameliorated with fresh blood, and the creature’s fangs sharpen; where the creature’s unarmed strike can be made with their fangs, dealing 1d6 piercing damage. Additionally, they are filled with undead strength, and their speed increases by 10 feet and they gain a climbing speed equal to their walking speed.

This stage of Sanguinare Vampiris can only be cured via the Wish spell or by a special 8 hour ritual which involves a 9th level Cleric, 5,000gp worth of holy water and powdered silver, and three consecutive castings of the spell Greater Restoration or Dispel Evil and Good.

  If the creature dies with Stage 4 Sanguinare Vampiris, they are immediately brought back to undeath as a Vampire NPC controlled by the DM, who has all of their previous humanity lost. This can only be reversed through means of a Wish spell.
